Use this page to report a suspected violation of policy, law or regulation, a security or privacy concern, or anything about Hei Atlas or the people who work on it that you do not want to raise by name. You do not need an account and you do not have to identify yourself.

What is, and is not, recorded

• Your report is emailed to the Privacy Officer. No IP address, browser details, account or session is stored with it, and the text is not written to any log.
• If you are signed in to Hei Atlas, your report is still sent without your account attached. For the strongest anonymity, open this page in a private browser window where you are not signed in.
• Do not include patient names, medical record numbers or other protected health information. Describe the concern; we will ask for anything further through a channel that is covered by the appropriate agreements.
• Nobody will be dismissed, demoted, threatened or otherwise penalised for a report made in good faith, even one that turns out to be mistaken.
• You may also report externally at any time, without reporting here first: HIPAA complaints go to the U.S. Department of Health and Human Services, Office for Civil Rights, at hhs.gov/hipaa/filing-a-complaint.
